Correspondence Study
CAVEAT
This Agreement is for the course titled: School of Nutritional Science
Correspondence Training Course. It is a legally binding instrument when
signed by the student and accepted by the school. By signing this Agreement
you acknowledge that you have read and understand its entirety. You also
acknowledge that you have received: 1) a written statement of the refund
policy including examples of how it applies (contained in the Catalog);
2) a catalog of the course including description of the material to be
taught and all material facts about the school.
WHEN YOU SIGN THIS AGREEMENT AND RETURN IT WITH YOUR TUITION WE WILL:
- Immediately sign the Agreement
and send you a copy as soon as we receive it.
- Within ten (10) working
days, send you your course materials via UPS.
- Give you a six month time
period to complete the course if you want a diploma. If you want a certificate
of completion instead, you may request extensions which we will certainly
consider.
- Receive your completed test
questionnaires by fax or mail, grade them and return them with your
results within 4-6 weeks.
- Grade your final test questionnaire
after receipt and immediately consider you for a diploma.
- Extend you an invitation
to become a member of the International Nutrition Consultants Association
(INCA).
- Be available via fax or
e-mail to answer questions you may have on the course material. Any
procedural questions will be answered over the phone.
ANY QUESTIONS OR PROBLEMS CONCERNING
THIS SCHOOL WHICH HAVE NOT BEEN SATISFACTORILY ANSWERED OR RESOLVED BY
THE SCHOOL SHOULD BE DIRECTED TO THE COUNCIL FOR PRIVATE POSTSECONDARY
AND VOCATIONAL EDUCATION, 1027 TENTH STREET, FOURTH FLOOR, SACRAMENTO,
CA 95814. (916) 445-3427.
This agreement is for the education service of the School of Nutritional
Science Correspondence training in the science of nutrition.
A total of 2 increments (consisting of 60 lessons) are required to complete
the course.
BUYER'S RIGHT TO CANCEL
The student has a right to cancel this enrollment agreement and obtain
a refund by providing a written notice to:

address shown above.
REFUND INFORMATION
This student has a right to a full refund of all charges less the amount
of $75.00 for the registration fee if he/she cancels this agreement prior
to or on the first day of instruction. Students enrolled in a correspondence
course shall have the right to cancel until midnight of the eigth business
day after the first lesson was mailed. This amount retained for registration
fee may not exceed one hundred dollars ($100.00).
In addition, the student may withdraw from a course after instruction
has started and receive a prorata refund for the unused portion of the
tuition and other refundable charges if the student has completed 60%
or less of the instruction. For example, if the student completes only
30 hours of a 90-hour course and paid $300 tuition, the student would
receive a refund of $200.00.

The school will also refund money collected for sending to a third party
on the students behalf such as a license or application fees. If
the school cancels or discontinues a course or eductional program, the
school will make a full refund of all charges. Refunds will be paid within
30 days of cancellation or withdrawal.
ACCEPTANCE SIGNATURES
My signature certifies that I have read, understood, and agreed to my
rights and responsibilities, and that the institutions cancellation
and refund policies have been clearly explained to me in their catalogue.

Code 94312 of the Private Postsecondary and Vocational Reform Act of 1989.

NOTICE: ANY HOLDER OF THIS
CONSUMER CREDIT CONTRACT IS SUBJECT TO ALL CLAIMS AND DEFENSE, WHICH THE
DEBTOR COULD ASSERT AGAINST THE SELLER OF GOODS OR SERVICES OBTAINED PURSUANT
HERETO OR WITH THE PROCEEDS HEREOF. RECOVERY HEREUNDER BY THE DEBTOR SHALL
NOT EXCEED AMOUNTS PAID BY THE DEBTOR HEREUNDER.
